The Buckaroo Foundation has grants available for physical therapy for your special needs child.
They believe that every child with special needs should have the opportunity to reach their fullest physical potential, and that financial means should never be an obstacle to that potential.
General Eligibility
- Residency: The applicant must be a legal resident of Baldwin County, Alabama.
-Focus Area: At this time, grants are specifically designated for Physical Therapy services that are medically necessary for individuals with disabilities.
-Mission Alignment: All requests must comply with the mission and vision of the Buckaroo Foundation to imrove therapy access and outcomes.
-Grant Frequency: Families may apply for and be awarded a grant once every six (6) months.
Your application packet must include:
1. Completed Application Form: A fully completed and signed application.
2. Proof of Diagnosis: Official documentation of a diagnosis or disability from a licensed medical professional.
3. Physical Therapy Records: You must provide a current invoice for the physical therapy services to be covered, as well as the most recent therapeutic notes from the therapist.
4. Submission Deadline: Applications must be postmarked by the 20th of the month to be considered for the following month's voting cycle.
Direct Disbursement: All awarded funds are paid directly to the licensed physical therapy provider.
